does this board come with the needed installation hardware (i.e. Standoffs and bolts) or will they need to be bought separately?

Mine did not, but I am pretty sure all that usually comes with the case.  I bought a new case when I got the MB and all the needed hardware came with the case.  Since my MB was used all I got was the MB and the IO backplate.  The MB works flawlessly.

Yes but the one I received was the cheapest of the cheap, probably a replacement shield. Made from cheap thin aluminum

I'm using a Xeon E-3 1230 v2 with this MB with no problems.

Yes. It has two of the high-speed slots so you can actually bridge two high-end graphic cards. Although, if you're going to spend that much on a video card, I recommend you get a higher-end motherboard, with a more recent high-end i9 CPU :-)

It will work, but will have lesser framerates because the 750ti is a slightly less powerful card.  medium settings sounds easily plausible for the specs you've given.

Yes, I would say that it is plenty good to hold your medium settings games. But some games like BF4 and AC4 I believe are CPU intensive, which you would make sure you would want to also have a good CPU for medium setting games. With my i5 2500k @ 3.3Ghz, 8gb ram, this mobo, gtx 550 ti (2gb), I can run those games on high settings without any problems.

Yes if going multigpu since its x8 x8 make sure you have pcie 3.0 enable chip ivybridge and enable pci e 3.0 within windows with the nvidia program or via registry tweak.  You can search online to see how this is done pcie 2.0 x8 per card is too little for high resolutions on multigpu systems.
