Do these come with screws?

Unfortunately no. You will receive 2 fan grills in a plastic bag and that is all.

140mm converted to inches is 5.5 inches, but when I measured these they came to 5.25 from tip to tip (not even from center of screw hole to center of screw hole!)

are these actually 140mm?

Yes, these grills are for a 140mm fan.  Consider that the outer edge of the fan casing and/or shroud will be 140mm (probably plus or minus 3/4 of a mm or so), therefore the inner cylinder and fan blades themselves will obviously measure less than this.  The grills are meant to cover the inner area such that any object larger than the gap between the grill's rings should not penetrate and hit the spinning fan blades.  Perhaps you SHOULD measure screw hole to screw hole, in which case the expected measurement for a 140mm fan mount would be approximately 124-125mm (~4.9 inches).
