Is this card 100% supported in Linux? Specifically, Redhat EL 6 or 7 or CentOS 6 or 7?

This card has drivers for RHEL 7.3 / CentOS 7.3, RHEL 6.9 / CentOS 6.9, Ubuntu 16.04.3 and SLED / SLES 12 SP2 for Linux.

what display adapters will I need to run 4 monitors since only two outputs are available? these monitors have both VGA and DVI inputs.

You can use DVI to VGA splitters, but will get the (2) channels twice. You might need to upgrade to the discreet output Matrox card.
