Is this router compatible with ARRIS SURFboard SB6121 DOCSIS 3.0 Cable Modem?

We have used this in many settings with different DSL, Cable (ARRIS models, though not with your exact model), and Fiber Optic routers.  Never had an issue.

We have always Plugged a LAN port from the Router into the WAN port (yellow) on the Netgear.

Does it include Modem as well? My apartments wiring has different slot like screwed one what else i need to buy

There is no modem. If you have cable internet you'll need a cable modem like the Motorola surfboard sb6141   I don't know what a "screwed in" interface is but if it is 75 ohm cable connection you will need a 75 ohm video patch cable for that and a cat6 jumper for the connection between the modem and the router.  Really it sounds like you might not be very familiar with network hardware. Ya might want to pay someone to set it up for you. There are a lot of setting on the router that you'll want to get right or you might end up providing internet access for the whole neighborhood.  Also most ISP's will require the modem to be registered before it will function.  So_ good luck.  Find a local expert or call the service provider.

This question was last asked over a year ago, so maybe things have changed... What version of the refurb'd WNDR4500 are they shipping these days? V1? V2? Maybe even V3??? Please include how recently you purchased in your answer. Thanks!

This was a refurb. Some hiccups in setting up but seems fine now. Needed resetting, turning off and on on a few occasions but the dual band is a great feature.

When an item is "refurbrished" what does that entail?

As a rule, "refurbished" means the unit has been returned to the vendor or manufacturer for a variety of reasons ranging from "purchased in error to "not what the customer actually needed/wanted", etc. In any case, the unit is tested to ensure it meets advertised performance. Deficiencies, if any are remedied, and because the unit can no longer be sold as new, it is sold as "refurbished" at a reduced price.

Does this item come with the disc to configure the router?

I don't recall whether the router came with a disc. However, the WNDR4500-100NAR N900 is covered extensively on the Netgear website. I rarely use a disc even if it is included as the software on the website is usually more current.
