Is this a two adapter pack or single adapter unit for $99.99? Do you get both the adapter to connect to a router and the adapter to connect to the device (tv, game system) for $99.99?

This is a two adapter kit.  One plugs into an outlet near your modem and the other you plug into an outlet close to the device where you want to get Ethernet.  It does block the ground prong for the outlet above it though, so you can only plug in a two prong plug.  I was surprised by this.

Question 2

Does this work through fuse boxes? My router and my computer are in two separate rooms at opposite ends of the place, with separate fuse boxes, but in the same condo.

It might, but it will likely work very poorly.

These have an indicator on the front that lets you know whether the two devices are able to reach each other and how good the link is. With a PC, you can actually check the link speed. But there's no way to know in advance whether or how well it will work in unusual situations.

My house has two fuse boxes and I have several of these throughout the house. One of them is in my cabana, 100 feet from my house and on a separate fuse box. It works reliably, the the link speed is around 70Mbps. That's better than WiFi would work over that distance without special antennas, so I'm pretty happy.

Question 3

Does this work with an iMac desktop? Also, do they require WiFi to setup or to operate? Thank You!

These will work with any system that uses a wired LAN connection.  WiFi is not needed for setup.  One connects to your router using a CAT 5 or 6 cable.  This will connect your internet through the power lines in your home.  The other acts as a receiver and takes the internet from the power lines and converts it to a standard wired LAN connection.  There is a small chance that the two devices may not "see" each other if your home wiring is on separate circuits.  I hope this helps.

I bought 2 of these kits.  My download speed can be 5-100 Mbs depending on where I put them in the house.  Unfortunately the upload speed is consistently 0.25 Mbs or less which makes surfing a little rough and gaming on it impossible.  This upload speed problem is even present when I plug my router and computer into the same PLA5456 using the 2 ethernet sockets (no other PLA5456 was plugged in).  Anyone else seen this problem?  It happens on all 4 units I bought.

I have not encountered this issue with my HomePlugs. Though I have not tested the speed limits, I have easily streamed 4K video through these to my TV while downloading something else on my computer. (One Homeplug connected to the router, another Homeplug connected to the TV and Desktop computer). I would suggest plugging 2 of these on the same extension cord and testing speeds. If the speeds are still terrible, you may have faulty Homeplugs

do the powerline have to be on same circuit?

Can't definitely answer that. I still use the powerline adapters today, even though my router has wifi which we also use. As long as the wall outlets go back to the same circuit breaker box, they should work. Having said that, in one room we had a hard time getting it to connect. Never discovered why. They should not be plugged into power strips. I love my powerline adapters and recommended them to my brother who was going to run cat6 cable.

Will these work with, or at least work without interfering with, a separate pair of ZyXEL HomePlugs that are AV1200?

Yes they will work not sure what you mean about interference with your existing plugs they will all work on the same network your older plugs will just have slower speeds as compared to the new plugs. Unless you have routed your network they all live harmoniously on the same network segment. I love them as they allowed me to eliminate my zyxel wireless AP for a few printers that never worked well wirelessly without the wireless bridge.
