Is this actually a PWM fan? With the 4 small PWM pins?

Mine came with both a MOLEX (male) and a PWM (3 pin female) connector attached to the same set of wires so I thing either connector can be used. I bought it for a friend so I can't comment on it's performance.

Does this fan include any mounting hardware?

It comes with 4 standard fan screws. It also has two styles of wire connections so you can plug it into your motherboard or directly to the power supply.

How long is the cable that hooks into the motherboard?

Right about 12 inches (30 cm). Also the end of the cable has both a 3 pin power and a molex connector, so you can choose to plug it into the board and cut the wire for the molex connector off, or you can use the molex connector and leave it as is. I'd recommend the first since the fan at max can get pretty noisy, setting it between 60 and 80% seems to be a better option.

The "Overview" section says this fan has an airflow of 53.65 CFM, while the "Specifications" section says it has an airflow of 65.16 CFM. Are either of these accurate?

I don't know what the CFM is but they work well
