Question 1

How tall/wide is the "grip"? Lot sof reviews claim it is rather small but I cannot find any dimensions anywhere.

The handle is 3 in tall, 2" from front to back where your thumb would cradle the handle to the button. It is 1 inch wide at the same place. I have large hands and I feel fine using this mouse. To be honest, I own about 5 of them. Yes, it would be better if it was a little bit thicker, but for most people it should be fine. Its not intended to be held tightly like a gun handle.

The real beauty of this mouse is the distance front you handle to the laser. You can alternate between fine and course corrections by changing the way you move the mouse. The scroll wheel with the thumb is pretty nice as well. Once you adapt to the handle you almost wish all mice had the thumb scroll. (I also use MX Master for this reason.) I spend 8-12 hours on it regularly with little to no fatigue. The only real problem i have with the unit is the faux leather grip on the handles will pop out after about 6 months of use. You have to super glue them back in and then they are fine for life.

This mouse is not good for gaming. It is great for large monitors and long use. I really wish Zalman would re-release this with a wireless version and buttons that don't need as much travel to click. There is so much space for additional buttons as well. Adding these features would make this the last mouse you'd ever need. 
Despite its design it actually has a very simple mounting time which plagues vertical mice efficiency.

Question 2

Has anyone tried to use this in an mmo like WoW? I'm curious how well the button layout works.

i haven't tried it in an mmo (been a while since i seriously played one) but everything other than the dpi change button is within very easy reach of your pointer + middle fingers and thumb so if anything it will be just as good as a normal mouse.

that said, the wheel scroll on this is stiff enough to have to be scrolled purposefully but not so stiff that it can't be easily scrolled so you would be safe to bind hotkeys to scroll wheel up and down but in the end it is still just a more comfortable 5 button mouse.

unlike certain mmo gaming mice with about 17 buttons or even the razor hex moba gaming mouse it isn't going to put any more hotkeys within easy reach than a normal 5 button mouse would.

in the end unless you're worried about wrist pain this isn't likely to help you out in an mmo like WoW, you're gonna want something with several more buttons in easy reach for that, but it won't hurt you either.
