what is the screw hole spacing?

not sure about the screw spacing when i looked up my model psu found that it was 135mm fan little odd ball if you ask me but everything lined up perfectly as long as the fan is 135mm if will work.

What are the volts and amps on this fan?

12V 0.35A as per the sticker on the back. They should really have the sticker side as one of the pictures.

What are the mount (hole) spacing of this fan?

The fan is a standard 135 mm power supply fan. According to my tape measure, the mounting screw holes are 4.25 inches apart.

Question 4

I need to replace the 135mm fan in my Raidmax PSU.  Will  this work, but the dead fan has only red , balck and blue wires ?  Red and Black are to power the fan, the blue must be foe the Led, which I really don't need.

Yes you just need to put red to the hot wire and black to the ground wire. I believe my original had three as well.
