Did this come in the blister pack with the disc? I bought else where and they just sent me the loose cable!

yes, and believe it or not, I have the old blister pack still in my hand happy to send the driver, though I believe it would be outdated at this point. I've had this almost 2 yrs

Is this compatible with Windows 8/8.1 64-bit?

I have only used it with windows 7 and it worked well. As long as there are drivers there should be no problem.
