Question 1

I have a case with a top mounted psu.  Can I mount this psu upside-down so the heatsink is facing down for airflow?

oof, these are designed to put all the critical components below the heat sink or otherwise out of the way of the rising heat.  Flip it, and you're trapping all that heat right where the components are.  You'd need an awful lot of airflow through this to get that heat our of there, and then what's the point of a fanless PSU if you need that many fans going.  
Better option isto get a case with a top mounted PSU and top vent, that's what I did with the Antec Solo II.

Question 2

Will this run with any R9 290 gpu?  My current set up is i5-3470, SSD, 2x HDD + 1 external HDD via USB.

Be sure you do not exceed the total wattage of the unit.  This is currently running in Phoenix AZ, on an outside shaded - most of day.  Home is air conditioned and the room is normally, in the 72 -82 degrees Fahrenheit.

Checked wattage at the R9 290 site.  It mentions a wattage rating of 250 Watts.  Only you know the total wattage of all your stuff.  So, be careful and ensure there is plenty of room in the compartment (CPU Box).  I am currently running - on my machine a GS800.  This has a fan and a changeable lighted color band.  It is very quiet and fan only runs when needed.  My suggestion - due to wattage of the GPU - you should go to a higher wattage power supply; be cooled with access to extra vents in the CPU Box.     Hope  this helps.

Question 3

How is it in summer? Doesn't it add more heat in your room? 50C seem kinda high for using it in hot weather.

I used it all summer long without A/C in the room and found it did not really contribute to the heat of the room. What little heat it did produce was worth the quiet it afforded. They only problem is the price.  I have two of these. One in my home in Florida and the other in home in Massachusetts. I am happy with both of them. They are dependable and quiet.

How many amp on the 12v rail?

I don't know. I never used it but it must be industry standard. I returned it when the Seasonic 400W fanless became available again. The Seasonic is flawless and silent. I have two of them. Don't know about the Rosewill.
