does it come with batteries in it?

Yes.  The batteries included will power 2000 watts for about 5 minutes.  They will power a 1000 watt load for about 16 minutes.  When discharged they will take up to three hours to recharge.  You can get more detail by looking at the load/time curves on the APC website.

Question 2

Can this be used with a standard 3 prong wall outlet? If not, is there an adapter that will safely connect it to one?

Probably not - this needs a 20 AMP input whereas most standard outlets are 15A, although some modern commercial facilities do have 20A outlets.  You can tell because a 20A plug has one leg rotated 90 degrees so the receptacle will have one leg slotted in a T shape so it will accept either 15A or 20A plug.  A quick image search for NEMA 5-15R and NEMA 5-20R will illustrate what I'm trying to describe.  In my experience the 1500VA units are the largest that will go in a standard outlet, and for the 2200VAs you need a 20A receptacle.  An electrician can convert your outlet if need be by just installing a 20A receptacle and maybe a larger breaker at the panel - it runs over the same wire.

Is it a brand new never used piece ?

I am pretty sure that if it is sold by Newegg itself, and it does not say anything about refurb etc it is brand new.

So check the description - if it is being bought from another seller through newegg then it is a question for the seller I would think.

The one I bought a couple years ago was new.

Do I need to purchase the Smart 2200 RM 2U battery separately, or is it included?

The battery is included with the UPS. Mine was not charged so it does need to be plugged in before powering.

Does the fan run continuously or only when on battery? How loud is it when it is just conditioning? How loud is it on battery?

Fan only runs when on battery. The fan is not loud. Much more quite than and of the three Dell servers in the same rack.
