Does this device come with only the rack mount option, or also the vertical stands/legs shown in the picture?

APC SMX-1500 is provided with both the legs for floor mount as show in the photos *and* the rack mount ears. You can use one or the other--or switch back and forth if you want.

Is this product the same as a APC Smart Ups x 120V Ext Bat Pk Rk/TWR? The manufacturer number I have is SMX120RMBP2U.

No: this listing is for the main SmartUPS device--which includes batteries. This device accepts an external battery pack like the one you list--which has only batteries and will not function as a UPS alone. You plug a battery pack into the rear of a UPS to extend the amount of time it will last.
