Yes, it does. 
I couldn't't use though. I needed one that is a telephony Modem.

Can this be used with Fios fiber optics?  They told me I could not, I think they are full of crap.  The cable is the same type of cable.

The answer is definitely NO, and here is why:
FiOS is delivered by fiber, that goes to the Optical Network Terminal outside of your house. From there, you could either pick Ethernet off directly to your own router (but prevents you from using FiOS TV services, such as Video on Demand) or use their typical setup where it is relayed by CoAx into your house. The signal on that CoAx is meant for multimedia devices, such as your DVR and other Set Top Boxes.

It seems confusing, because their own router connects via CoAx too, but it is NOT a cable modem, it is using MoCA. Their router is a critical part of their television services too, because if you disconnect ONLY the cable connection to the router, the STBs malfunction almost immediately.

Anyone just getting solid white lights for power and link instead of solid green lights like the pictures and manual depicted when the modem is fully powered on and connected?

Yes...I also get solid white lights...so I don't know why Arris says green??? The lights are white.

What is included with this listing?

Mine came with everything you'd find in a retail package. Modem, power adapter, Ethernet cable, and an owners manual/quick setup guide.

Question 5

We used splitter to hook up a modem for internet phone from Time Warner Cable, and having issues.  No problems until we hook up the phone modem, then irratically shuts down internet.  No problems when not attached.  Any clue how we can make this work?

Good afternoon,

I'm happy to help, what signal are you splitting, the Coax or the Cat5e cable?

If you are using a Coax splitter, make sure it has the frequency requirements, most will say 1100mhz or 2.4ghz either one will work, although I'd opt for the higher frequency range myself.

If you are using a Cat5e splitter, you'll need to use a network switch to make it work properly. Netgear & Cisco/Linksys make excellent network switches (4 port is usually the minimum.)

Connect the Cat5e from the modem, into the network switch, then connect the computer and the VOIP phone to the network switch as well. That should stabilize the connection.
