Question 1

Do both SATA connectors have to be connected to the motherboard?  Or can both drives be connected to the mother board using a single SATA connector?

There is a single SATA power connector which operates both drive bays but two SATA data cables will need to be connected to access each drive in the bay when using two.

Question 2

Are the dimensions the same as a 3-1/2" drive? I want to use it in a hot swap rack!

Yes,  and it does come with 4 screws and 2 of the little triangle key for the locks.  I don't use the locks, since I'm the only one that has access to the computer this is installed in.  So far, I've had no problems.  I use the lower bay for an extra HD, and the upper I've swapped in both HD and SSD for back up storage.  
Hope this helps. PS, I used this in a case with only 5.25 bays and had to use a 5.25 to 3.5 slot open faced adapter.

Which does the small switch on the back of the rack control exactly, #2 SATA only or both SATA interfaces? Thanks.

I don't have a definitive answer for your question.  The project this adapter was to be used on, got shelved. So I was able to take out the unused unit and pull out the back plane mounting unit.  The circuit traces form the switch looks to control both LEDs even though by the design the switch is associated next to the SATA 2 data connector. And the wording in the description (poorly) states the same.  Since it wasn't installed I never actually worked with it with drives installed.  Best Regards
