How long is the PWM fan cable?

450 mm long I believe, I still use a splitter that's about another 450 mm approximately, they are not long enough for a mid-tower, probably reach the closest header with one but it would have to go right through the center of the case not around the back and through grommet, most fans will probably be the same give or take depending on the depth of the case.

WHAT IS THE DIFFERENCE BETWEEN THE ONE w/PWM VERSUS THE ONE wo/PWM?

The difference is the amount pins on the connector, with pwm has 4 pins, without pwm has 3 pins, the first answer second pins have the power, the third pin reports back the rpm's, the 4th pin is to control the revolutions, but on modern motherboards the bios can control fans with 3 pins not as precise as with 4 pins, you could either set the volts going in the fan or fans or some other type setting, hope that answers you're question.

can this be run on only 2 wires?   + and - ? I dont need power management.  which wires do I use if so?

Not sure what you mean but I run it directly into the mobo using a fan splitter
