Is this just a case with a steam download code in it or is it a actual disc I need to have in my drive at all times?

The disc is there to lower the amount of data you have to download. It also comes with a Steam code that you have to redeem on Steam to activate the game. You can skip the disc completely and simply download the entire game from Steam (by entering the Steam code) if you want to.
