Does this game required to have a steam account to download this game?

Yes, you will need a Steam account. Additionally, the full game isn't on the disc, only a portion of it. you will have to log into Steam to fully download the game plus any day 1 updates.

The PC version is released in December, while the Xbox and PS4 versions release in November.  Does the PC version really come out a month later?

Bethesda announced that Fallout 4 will be released on PC, PlayStation 4 and Xbox One on 10 November 2015.

Does this come on a DVD, or is it digital download?

DVD with small portion of the game on disc.  The game takes 30 g of HDD and a DVD only holds 4.5g so the rest will need to be downloaded from steam.

Does this game require the Fallout 4 season pass for full access?

No, Season Pass is just a discounted prepay for all DLC to be released in the future.  Word is Bethesda has no plans for any DLC till late 2016.

I don't have an optical drive at all, but this is part of a combo so I want to buy this copy. My question is, can I download the entire game after activating the purchase in Steam, or does it require the first few gigabytes to be installed from the DVD?

I do not have a cd drive on my system; does that matter since i have steam? Does it come with a product code to activate on steam or does it need to be physically in the drive?

It won't matter. the disc itself only contains a little over 5 gigabytes of the game.

If you have Steam, then just select auto-update. I'm not sure about non-Steam systems or just the stand-alone game.

Yes - This is available directly from Steam.

When you purchase from NewEgg, the copy you receive will push you to have a Steam account any way.

It's a pretty good version so far! Graphics are awesome!
