Why is this classified as "HDMI to HDMI" instead of "DVI-D to HDMI" (or vice versa)?

Because, "HDMI" is being used to describe the purpose/use of the cable here.   HDMI is an industry acronym standing for high definition multimedia interface,  which refers to a device communication of uncompressed video and/or audio data from one device to another such as a computer to a television, monitor, projector etc.....

Can i use my ps4 on a screen with only dvi with this cable?

yes but the hdmi sound from the hdmi out from the ps4 is going to go no-where. this cord can be used like that but no sound. it's mostly used for being a 3rd monitor cord on higher end graphics cards that can do 3 or more monitors. The other two being either 1vga/1dvi or 2dvi connectors. So it's how most people add the third monitor per graphics card

Its male to male so it can be used an HDMI output to a DVI-D input, or a DVI-D output to an HDMI input.

There are many different kinds of ports for display. This one is a DVI to HDMI male to male cable. So this can directly connect from a DVI port of a monitor/PC unit to a PC/monitor unit. This is not for VGA which is a different type of port. If you really want to use this cable, you need to buy an additional adapter for it and that require some other attentions to determine depend on what you want to connect with.

I have Dell OptiPlex 3020 - Need to use that with Dual monitor - Have the VGA cable for first monitor - not sure if this is the right cable for the second monitor?  Please advise.

If you don't have a video card and are trying to use the motherboard you have to change the setting in the bios.

Sorry, I don't use mine with a KVM. It was just a super simple way for connecting Digital to HDMI and has worked very well with my configuration need.
