Can someone provide a label photo or specifications of the included PSU? Thanks!

I actually built them for a customer. I no longer have access to them but what are you looking to do? Maybe I can help you figure it out

Where do you get riser cards? The specifications section says consult manufacturer's web site for riser card info. I did that, and identified the manufacturer's p/n DD-655-C5-02. However, Newegg does not carry that p/n and I have been unable to find anywhere else that carries it.

Just order it directly from iStarUsa, they are ok with sending parts. Also note that it's a ribbon riser, I'm pretty sure any other ribbon riser would work as long as the mounting holes match.
