Does the tachometer lead wire extend to both fans or just to one of them?

If you are referring to the sense and control wires, they extend to both.  As long as you connect to a PWM header and use matching PWM fans, it will match both fans to your controller's settings.

How many fan splitters come in a box? The picture above shows two separate splitters.

I'm attempting to add another fan onto my CPU cooler, will these cables work for a CPU fan?

Yes, the Noctua NA-SYC1 4 Pin Y-Cables can be used to connect and control additional fans, such as a CPU fan, via a single PWM fan header on the motherboard. The cables allow two 4-pin PWM fans to be controlled simultaneously.
