Will any old ATX power supply work in this case, or is there a specific fan configuration needed to ensure the PSU gets enough air?

The case take a standard ATX power supply no idea what size that is. I use a Antec EA-380D Green in the case Dimensions: 3.4" x 5.9" x 5.5" and it fits perfectly. Make sure the fan on the power supply vents to the rear and not out the front. In this case if you use a ATX power supply it sits at the front of the case so you want to make sure the hot air is not vented into the case.

Does this come with rails?

No the rails are not included with this case.

If I install a 2U power supply, will I be able to access its switch without opening the case?

it has an external power switch but that is supposed to be hooked up to the mother board. this is a server case. the power supply it requires would turn on when the mother board is turned on.
