Is this a download, or a disc?

This is a full box, with physical media inside. It can also be downloaded digitally as well. All you REALLY need is the code on the outside of the DVD sleeve. Once you have that, you can decide to install the physical media or just go to Blizzard's website (battle net), register the code and download the game digitally.

There is likely an advantage to downloading digitally because the game on the physical media is way out of date at this point (11/2013).

Question 2

Does the game come with CD or just one online code?

The box contains one "Game DVD", so you'll need a DVD or Blu-Ray optical drive to install the game from the disc. But I can't imagine any computer able to play the game that doesn't come with one. THEN, the installer will go online to download the newest update to the game, which will probably take longer than the disc install.
