Do I have to keep using the DVD to play the game or the DVD is just for initial installation?

You don't even need the DVD at all. What you can do is when you buy it, on the CD-case comes an install key and you can just use that to activate your copy online.

Both. Has the DVD in a sleeve labeled with the Serial Key.

Does it still bring the Noire Widowmaker skin right now (even though it was a pre order bonus)?

yes, Most of the widowmaker codes were already pre-packaged in the box. I got mine after the pre-order date and still received a code.
