Does this product include an AC power supply?

The simple answer is no....
The handset can be powered with Power Over Ethernet, or an optional wall-wart power supply which is also available through Newegg.

Does this unit include the power supply adapter?

No you have to purchase them separately.  I made the same mistake and ordered them not realizing this.  These also work with Power Over Ethernet POE so Cisco did not bundle a power adapter.

I want to change the title of the station on the phone. For example, it says MANUFACTURING DEPT on the screen savor. How do i change it?

This is an interesting question.
I am not sure what you mean by "screen savor"
If you bought the phone new, I believe there should be nothing there.
Generally speaking, the phone is populated with information from the IP service provider when you assign the phone to your provider.  This is where the phone user, phone #, and other information in the phone come from.  This is where you would change the info.
alternatively - you can go to the phone control panel by typing in the IP address of the phone from a browser (assuming the computer is on the same network).
You can find the IP address by pressing the menu button and then 9.  There are many options inside the phone control panel for managing the phone.
