Looking for A/C powered, is this one?

fan plugs into computer as cooling fan. it has the simple universal plug powered by computer. but i'm actually using this fan as cooling unit attached to a small 5W solar panel.

Does the smoke colored fan have LED's on it?  I am trying to avoid LED lighting on my current build.

The CM sickle flow fans can be bought with or without leds. You just have to make sure you buy the ones without.

can it be used under 230 volts? thank you

I used a Cooler Master V10 CPU cooler over clocked on an Asus Rampage II Extreme with good success.  The system was powered by 110 volts without a problem.  One of the SickleFlow fans over a period of time slowed down and I was able to replace it through Newegg.  Hope this help you.

Does this fit as an add on for 212 evo? (i.e. to pull air from opposite side of CPU cooler fins)

it could fit if you have the mounting brackets for the other side of the heatsink. otherwise you have nothing to mount the fan to. keep in mind that you need to mount the fan in reverse.

I've seen mixed reviews, is it quiet or not?

The best answer is a firm maybe; my experience is that every single fan (out of 5) had a both an audible tick (motor noise) on every revolution, and poor sound performance given the real airflow.

If you google "120mm Radiator Fan Roundup Part 2: Fan Harder" you will find an excellent article on Anandtech that includes this fan, it is at the bottom of the thermal performance.
