Do I need a controller to use this game? What kind?

Nope. You can certainly use one, but I find the lag of wireless controllers to be too noticeable (i.e. I die too much).  I prefer when piloting helicopters, but that's about it.

I use a wireless xbox 360 controller if I ever choose to use one, but I much prefer keyboard and mouse.  Get yourself a good mouse with on the fly DPI settings (if you don't have one) and a nice mechanical keyboard that can handle any abuse you dish out and you'll be a happy virtual soldier.

does this game allow off-line individual play?

If you're talking about the campaign/singleplayer than yes. You can either start the campaign from Battlelog or from the bf3 exe file in the game directory. If you mean you don't have internet access but still want to play a multiplayer match by yourself than no, you cannot, you can only find and start a multiplayer match from the battlelog website.
