How well does this handle games like Fallout 4 on high settings?

The card works nicely in Fallout 4 on High Setting and probably in Ultra High as well. In fact, I'm running the game in 1440 p (with an ASUS ROG) monitor on High with several setting in Ultra High and with about 40 Nexus Game Mods installed which take up a ton of memory but the effects are fantastic with green grass and all.  With all the of that I run about 50-60 fps in open country and up to 120 fps in indoor areas.  Awesome card for the buck and it will get me through until I upgrade to a pure 4K gaming system.

In the game Skyrim with about 60 mods consistently keeping over 90 fps on 1440 p Ultra High Settings.  Great price points.

What kind of framerate can i expect with a single one of these cards, an i5-4690k, and a 4k ips monitor with only a 60hz refresh rate?

Your actual frame rate is entirely dependant on applications running and number of displays attached.  While the card can output 60Hz or better class video for movies etc, the real frame rate issues are about rendering speed, GPU/CPU loading, GPU/CPU memory limits and selected resolutions.   Run one game at low-res and you can expect SUPER high frame rates, run 3/4/5 monitors at HD/4K resolutions while ripping DVD movies and you can expect horrible frame rates.

I have a cougar SL 400 with one six pin connector will this work if I use a 1x6 pin to 1x8 pin adapter? Or do I need a new power supply?

Your Cougar SL 400 has two 12V rails. One rail is 15A and the other one is 16A. The GTX 960 is a 20A card. When you plug in the PCI-E connector, it's using either a 15A or 16A rail.

You should upgrade your PSU for best reliability and end results. You could use the card, but if it attempts to pull maximum amperage, the PSU will not be able to accommodate the GPU requirement.

Single rail PSUs gained in popularity for this reason, especially in the smaller range where there is not a lot of power to split up.

I would try for a EVGA supernova G2, 7 year warranty, 550w, +12V@45.8A, like $80:
EVGA G2 550w

If not maybe SeaSonic S12II, 520w, +12V@40A, $60, it's SeaSonic:
SeaSonic S12II 520W

The cards come with adapters and what not, it's the specs on your PSU that won't measure up technically, not the connector factor.

Question 4

I have an i5 3570 cpu. Would I have to worry about bottle necking with this GPU?

I currently run 2 of the 4gb versions of this card with the i1 4900. 
My research found that the graphics card will perform most of the graphics calculations taking away from the CPU usage. While gaming at approximately 72 FPs my CPU usage is at 20%

Some users have reported their cards becoming bricked after updating to 364.72 Nvidia drivers, and others like me just had Windows Aero not work and report a desktop error so reverted to 347.88 drivers. Will these Nvidia cards be covered under warranty if they get destroyed from bad drivers?

I am running 364.72 right now and haven't had a problem. I'm on Windows 10. 
But I also agree that you don't need to update drivers unless the new one has something you need. Ive had trouble with new drivers before.

can this card runs triple monitor? if so what slot should i use and should i use any kind of adapter for it to work?

Yes it can. You will need to have the appropriate cable or adapter for your choice of configuration. I use the two display ports and the hdmi for my 3 display setup.

Is this card UEFI Supported.  In other words can I disable the CMS in the M/B UEFI BIOS to enable Fast Boot or Ultra Fast?
