will it operate with windows 8?

No idea... purchased a bunch of these from New EGG and several days later get an email that they are out of stock.  Refunded money.  When stock finally arrives, the price jumped up.  A hassle and a scam.

Fujitsu website says this FI-6110 comes bundled with Adobe Acrobat standard, but that is not listed in NewEgg literature. Does yours include a copy of Adobe?

It included Adobe Acrobat standard version 9.  That is not the current version of Acrobat.

Can it scan both sides of a document? Does it have double-sided scan ability?

Yes it does. This is a good scanner for the money. I have 2 of them that I use at office with Nuance, Omnipage Ultimate scanning software.

Can this replace 6130... Does it have the same properties....we use it in a medical office and have 6130 but need another scanner?..

This model actually replaces Fujitsu fi-5110c. This will work fine with your Medical Office EMR/PM System since this is also Twain Compatible. This will be slower than a fi-6130.
Fujitsu fi-7160 replaces fi-6130 & fi6130z

Is this scanner TWAIN-compliant? Will it scan checks directly into quickbooks desktop software?

Scanner is definitely TWAIN compliant and is a reliable workhorse.  QuickBooks question will depend on version of QB software and configuration but if the only requirement in QB is TWAIN compatibility, this scanner will handle that as well.  I'm pretty sure the scanner ships with OCR software as well or it can be downloaded from the Fujitsu website.

Can the DPI be set to 200X200?

We set it to 200 dpi in quick-books. Other than that, we just leave the settings right where they are at. BTW, this is an awesome scanner. We bought 6 at one time and have had zero failures other than operator failure.

Will this scanner scan 11 x 17 paper?

I cant paste the link, but google fujitsu fi6110 specs
and look at paper size.  I don't see that 11 x 14 is supported.

Can you scan to USB. Review says cannot produce PDF.  Is that correct?

The software included with this scanner can indeed scan to PDF, just make sure you install it!

If the surface tablet is a Surface Pro 3 that runs a full version of Windows 8 / 8.1 it will support it.  If it is one of the older models that runs Windows RT it might not be officially supported by Fujitsu on that operating system though it still might work.
