How many concurrent outputs can this card support?

With 4 outputs it CAN handle 4.  Now if you are trying to run games across 4 screens you would need to tune the settings relatively low opposed to if you were running on only one monitor.  Now if you are looking for multiple monitors for productivity use (trading/spreadsheets/etc) the one card would run up to 4 monitors fine.

You need a compatible display and video card to support 3D. As long as you have those two things, you should be golden. The monitor and video card should tell you what kind of connector the HDMI card needs whether it be a regular or a min type HDMI connector.
