Does this come with the SATA cable and the power cable?

No, only the hard drive comes in the box.  You'll need a standard SATA cable, which usually comes with your motherboard if it supports SATA.  Also, most newer power supplies have connectors to power SATA drives.  If not, you'll need a "Molex Male 4 Pin Power to 15 pin SATA Female Adapter Cable."

Question 2

Is this thing really that loud? Does a sound dampening case do anything about it?

The one I bought isn't loud at all. It's mounted in a pretty much open case, meaning I have a lot of perforated panels for fans and honestly, I don't hear it at all. Just hear the purring of the fans. Perhaps you have a defective drive or perhaps maybe placing some plastic or rubber washers at the mounting screws would help. The drive shouldn't be all that loud though and shouldn't need sound dampening of any kind.

Do you have any of these drives with a manufacture date of AUG 2011?

I am not completely sure of the manufacturer date.  In the past I have never had a problem with Hitachi drives but the one I ordered failed not too long after I put it into a machine I build.  The vendor, goharddrive dot com (through Newegg) is in the process of replacing it.

Is this drive new, used, or refurbished?

If these drives are new, then they're new old stock because the 7K3000 series is 2 generations old and was available even back in 2011.  It's very common for used and "refurbished" HGST drives to be advertised as new.  HGST's newest non-helium desktops are the 7K6000 series, but in 2016 many dealers were still stocking brand new 7K4000 drives.  There was no 7K5000 series.

Does drive use jumpers for master slave set up?

No. Master and Slave is an IDE thing, these are SATA drives. As long as your motherboard has a free (and working) SATA port all you do is plug it in and voila. There are no jumpers to worry about.
