Does it include a Windows install CD?

No, there is no Windows CD included, however you can make backup DVD's s well as a Repair DVD when you first set up the PC.

Question 2

Does this computer actually have 2 ps/2 ports for keyboard and mouse, 1 parallel port for printer and 6 usb 2.0 ports on back, an Ethernet port and 2 usb 2.0 ports on front as I have seen in pictures of this computer on other sites?  Also is there an empty 3.5" drive bay in front? How many empty card slots?

I don't know about the PS/2 ports for keyboard and mouse.  It uses USB ports for them though. There are 6 USB ports on back and 2 USB ports on front.  Yes, there is an empty slot below the HDD on front left side. I don't use the parallel printer port but it's there.  Everything works great, I recommend it, you really can't beat the price for Win 7-64 bit, 1TB HDD, 2.33GHz speed and 4 GB RAM.  Recommend you Google for "HP DC7800 specs".

Bought this computer for some heavy-duty photo and video editing and want to upgrade to a 2GB 128 bit video card that supports a 3 monitor setup, but I'm not sure if I can use either an AMD card or an NVIDIA card because I know some AMD cards provide multi-monitor software, but I don't know if NVIDIA cards to the same?  Help me newegg community!

Honestly I wouldn't think it would work in this machine, it's so old.  I did a Google search and found some intuitive results "Nvidio supports 3 monitors".

can i use 2 monitors with this device?

There is only one VGA port on the unit I received.  To use two monitors, you would probably need to install a second graphics card, and since it's a small-form-factor case, you'll need a low-profile type of internal card or an external USB-to-VGA (or DVI) graphics adapter.
