Question 1

Says refurbished what grade are they rated? Scratch and dent what can I anticipate at delivery?

The monitor itself is extremely sturdy, very heavy with metal parts and very thick heavy plastic base. There were no scratches on mine however the silver metal piece on the bottom of the front (just under the screen) was bent at the corner and pulled out about an inch from its original position. Since it was metal I was able to bend it back into place. So to answer your question, Yes there is a chance the one you get might have some cosmetic issues. Functionally its great, its my favorite monitor out of the 12+ that I have used in my office.

Question 2

It says in description DVI-D while the image shows VGA. Does it come with a VGA Cable or DVI? and What plugs are able to be plugged into it?

It has a DVI socket but my screen came with a VGA cable with an adaptor so it worked out I guess.. Barely inconvenient, but other than that it's a pc monitor, nothin special, nothin awful :/

Question 3

i dont know what pivoting, tilt, etc, imply exactly.

can the monitor be rotated by 90 degrees so that instead of it being "wide" screen its a "tall" screen, per se?

Yes, it does turn 90 degrees to portrait mode.
