Does it come with a cartridge?

It does come with a toner cartridge.  As with most laser printers, the toner cartridge has less toner in it than refills.  We've had our printer hooked up for about 2 weeks and the starter cartridge is still working, but we are light users.  I'd suggest buying a replacement toner cartridge at the same time you buy the printer, just to keep from running out of toner at a bad time.  You'll need it eventually anyway.

how many copies are done from a cartridge, what size cartridges does this machine use and cost?

The toner last time i checked it was about 60 USD. But a laser printer can print a lot depending on what it is. Now if you are printing a lot of images you will use it faster.

The FAX functionality is confusing, where on the machine can I determine the status of a fax and sending in a fax---
 I need a fax confirmation sheet with an update on the status of the fax:  "successful fax, line busy etc....."?

I'm don't believe the printer gives you fax confirmations.  There is a setting under Main Menu "Fax Job Status" to let you know if you have pending faxes.

The all-in-one unit is a laser printer.  It uses a toner cartridge.  Toner looks like black talcum powder.  We've had our unit for 4 months and have yet to replace the starter cartridge that came with it.  You can find toner cartridges for all kinds of prices.  Look up the toner cartridge it uses to find prices.  You will find that, even if you pay list price for HP toner cartridges, it will be cheaper than using ink cartridges.  My other laser printer, which is 10 years old, went through 3000-3500 pages before needing a replacement cartridge.
