Does this card include "in-box" drivers for Windows 2012 server?

No, but server 2012 and r2 detects it out of box. Windows will also update to lates greatest. Also you can just download the Pro 19.0 or i think its 20.0 now from intel dot com / downloads

Cannot tell you the exact part number, but we know it's Intel Pro 1000BT series and drivers
