Is the fan on this cooler quiet? (one reviewer has already said their's was loud as a con, but no one else has mentioned it)

For those wanting an industrial strength fan, this is the right fan.
It comes with the price of being on the noisy side . . .but in an 
industrial type data closet (the target audience for this type of fan) 
 . . .who really care.

If you are noise sensitive then this fan is not for you.  Since the 
average PC  has their PC in an exposed area and usually within
2-4 feet of their seat it will not fill the "quiet" need.  
But, if you are not noise sensitive, it could fit for you.

Question 2

Will this work with LGA 1200 i7 or i5 processors?

No, the Intel Thermal Solution BXTS15A Heatsink is compatible with Intel LGA 1156, 1151, and 1150 sockets. It is not specified to be compatible with LGA 1200 sockets.

Question 3

Does anyone have a wiring schematic in this I need the voltages for troubleshooting?

It's a standard 12V PWM fan - Black ( let's call it wire 1) is Ground, Yellow is 12V, wire 3 (usually yellow) is signal speed, and the 4th wire is the control pulse.

Will this heatsink work with Intel LGA1366 on an ASUS rampage III Formula board?

It looks like the same one I have and it works I've had it since July 2017. I have a maximus formula IX which is a LGA 1151 I don't know if that's the same size.  I believe it is smaller in size so no.         socket B vs Socket H LGA 1366 is a dead end LGA 1155 is still current.

Question 5

the specs on the web page say fan fits 1150,1151,1156 processor types. I just received product and box says 1151. No mention 1150 0r 1156. I have 1150 and before I open box can someone assure me it will fit ?

heatsinks for sockets LGA 1151, 1150, 1155, and 1156 are interchangeable as they all have the same distance of 75 mm between each screw hole.
