If the Galaxy S3 says MicroSD for the card type, then it will fit, in terms of physical size. As all MicroSD cards and slots are the same physical size.

If you are asking whether the Galaxy S3 will accept a 64 GB size, according to Samsung, it will accept the 64GB size, but you may be limited to 32 GB.

Kingston is an excellent brand class 10 memory is the fastest that I've found and I believe that it will work for your phone being that it's not that old and if for what ever reason that it doesn't newegg has an excellent return policy

I do not remember.
I did not need an adaptor, so I paid it no mind.
I will tell you it was a defective piece and should contact newegg to replace it.
Recently I purchased one made by Transcend, which did come with an adapter.
