I'm having major, major issues with this monitor. It works great for 4-5 days, then refuses to 'power on' from sleep. LG originally helped fix this by switching the power cord from a surge protector to a direct wall socket, but now the same issue happened: the computer tries to 'wake up', the white power indicator light comes on, but the monitor does not show backlighting or any acitivity whatsoever; it just goes black. Pressing power button, disconnecting and reconnect cables... nothing makes it work. I have a desktop PC with a GTX980 and nothing is malfunctioning... I cannot make sense of it. Does any have any ideas???

One thing that is odd:  You mentioned that LG originally helped fix the issue by switching the power chord from the surge protector and plugging it directly into the wall socket.  Did this work for a while (hours, a few days) but then the same issue happened again?  If so, It sounds like an issue with either the power chord on the monitor or something with the monitor itself (something related to power...not "receiving" power...it's dead).

It COULD be the power socket but I thing that would be unlikely.  You could test this easily (obviously) with another monitor or some other device.  You can test everything as I suggested in my other reply just to be sure but just based on what you have said...that you plugged it in directly to the socket and it worked and then failed, it sounds like a power issue...more than likely the monitor or cable.  MAYBE the wall socket.  You just have to test each related item and figure it out by process of elimination.

what all does the package include? obviously power cable and monitor. but any displayport or hdmi cables?

It definitely came with a displayport cable. I believe it also came with an HDMI cable as well but don't quote me on that one ....

If you have two active inputs and disconnect one, does it automatically switch to the other one or do you have to manually change the input?

Does this monitor require a driver disk?

There is a disk included in the package but I did not find it necessary to use it as of yet. Tuesday evening, November 21st 2017,  I just hooked it up and started using it. There is, as you may know, a 5K screen which you can purchase. Although this one works great and is discounted if you use the Amex card.

Does this monitor have flicker free and low blue light mode?

I would say, yes it does.  Based on the following table from LG web site:  ***NEWEGG WON'T ALLOW ME TO PUT A LINK TO THE WEB ADDRESS HERE!***
If you click on "Compare" for Model 27MU58B, a RESOLUTION table is displayed that shows Video HDMI:  3840x2160@60Hz (Ver 2.0).  The actual cable that came in the box is Part No.:  EAD00926103CHL71PC413.  A search of the LG Web site gets no results for this Part No.  You might call them and ask about this Part No.

Does this monitor support true PIP, e.g, on-screen display from two inputs simultaneously?

I have this monitor, but haven't tried PIP. On LGs support PDF for this monitor, it says: "Its advanced options have expanded to
include 4 types of Picture-in-Picture(PIP) Mode among 14 options. With PIP
Mode you can work on the main screen while watching a video in a smaller
window floating on the screen"
