this looks like the fan would be blowing upside down into the case?

It depends on the case. In the case I have it in, the fan blows out the bottom of the case. In a different case it could blow out the top. None the less, it is a killer power supply.

does this have a fan for the rear?

It certainly has a fan, but it doesn't face out of the computer's rear, where the on/off switch is. It faces the interior of the computer case.

how long is the cord that goes to the grounded outlet?

Hey Peter, I have to apologize to you, because I bought this power supply for a computer that I built for my Dad. I do not remember how long the AC power cord was. My Dad has not had any problems with the power supply so far, and it has been over a year now.
