Is this a desktop or a mobile mouse?

This is a full size desktop mouse but I have found it very easy to drop into my backpack and carry back and forth between my office and home everyday, so I would say it can function as both despite its size.  Just be careful of the bottom battery cover.  It can come off easily.  But it has the best battery life of any wireless mouse I have ever used.  I have two of these and love the product.

Absolutely. I use Windows 10 with this mouse daily. Every function works. Battery lasts several months.

Does it come with batteries?

Sure does. Install the batteries, plug in the tiny USB receiver and turn on the mouse. It was immediately recognized by my Mac. Just as a side note there's a small button just behind the scroll wheel that, if you push it down, gives the wheel some traction for scrolling. Otherwise it's free scrolling and can take some getting use to. Great mouse for the money. Slightly smaller than most mouses. Great product.

I need a mouse that will perform well on a hard surface...probably without a pad.  Is this the best fit for that?  It will be used by a radiologist that will require heavy use of the wheels as well.

I use mine on top of my (fake wood grain) computer hutch all the time, no pad; never a problem at all.  This item works very well on hard surfaces.  Once I had it on something very reflective and smooth that happened to be sitting on my desk (sorry, can't remember what it was, exactly) and it amazingly worked just fine on that, too.  Best mouse there is, IMHO.

Does anyone know if I can bind the keys to specific actions, particularly for gaming?

Technically yes you can. Using the logitech setpoint software it does allow you to customize what every single button does but I have not personally tested this feature. I use a lot of the default settings and set up a few buttons to do extra things like open a program or zoom in or out
