Does the controller come with a USB cable?

No, but you do need one to update firmware.

I also have had some terrible and we'll documented performance issues with Bluetooth on multiple machines with different Bluetooth adapters so I've given up on that and just use it via USB all the time.

Is this the controller from xbox one or xbox one s? I want to know if it has the extra features.

It is the new controller from the One S. It includes Bluetooth to connect to Windows 10 devices.

Does this one have 3.5mm?

Yes, if you check the features description, you'll find the following text:
"Plug in any compatible headset with the 3.5mm stereo headset jack."

Do I need to buy additional items in order to use this on Windows 10? [e.g. Bluetooth Dongle or the like?] Or will this work, wireless and all, with a Windows 10 PC?

It connects to your Windows 10 PC via Bluetooth.  If your system doesn't have Bluetooth, you will need a dongle.

What kind of battery does this controller use? built in rechargeable? AA? AAA?
I had old Xbox controller and battery pack/case if it's the case with AA/AAA stop functioning after few months and it seems to be a known issue.. what bout this one?

This controller out of the box uses AA batteries. Charge kits are sold separately, made by a variety of companies, and range from $10-25.
