Will this work with an XBox 360 PC controller (my original wireless receiver is dead)?

This adapter only works with XBOX ONE controllers. You'll want to either purchase another XBOX 360 controller adapter to use your current controller/s, or an XBOX ONE controller alongside this new adapter.

the images show 2 different adapters... is this the smaller adapter or the larger adapter?

The little one, well I got mine and it was the little one but I also made sure that mine was "Sold and Shipped by Newegg" The one that is currently being displayed is Sold and Shipped by Antonline so there is a possibility that you may get the larger one. Here's a cool combo I found, not bad for the price I think but the best is that it's Sold and Shipped by Newegg so no need to go to anyone else other than Newegg for RMA's and Claims and I received the small one from Newegg so I would assume they will send you the smaller one as well. Item# 74-103-656, just type that or copy and past it to the search bar, hopefully this helps.

will this connect to headsets as well?

yes it will, i have mine connected to the Arctis 9x.
