Does this board require a video card?

This motherboard does not require a video card. Most people put video cards in desktop computer builds to provide better graphics performance. It depends on what you will use it for. If you plan to play games then I would suggest getting a video card. Like I mentioned though, the video card isn't a requirement. Just hook up your monitor with HDMI or DVI.

Which sata ports are the sata III (6.0 gbps)? I would imagine it is 1 and 2, I just wanted to make sure. Any help is greatly appreciated!

Yes, you are correct.  The other ports are 3 Gb/s.  If you want all of the ports to be 6 Gb/s, then you would need to purchase the H87M-E35.

The only digital audio output that this motherboard offers is through the HDMI port (along with video of course since HDMI does both).

if your just simply swapping motherboards the answer is. NO.

The board you stated in your question is so outdated about the only thing you could use from the old computer would be the case.  Literally everything else would have to be replaced as well.
