So, this card does NOT include a cable adapter from 12vhpwr to 4 x 8pin cables?

This card DOES include a 4 x 8pin adapter. At least mine did. But from what I understand the 4090 only needs three inputs and would even work with just two. For example, my PSU vendor (Seasonic) provides a 2 x 8pin cable for 4090. I opted to plug in all four, but I don't know if it's necessary.

It's a mid-tower, but it has E-ATX motherboard capapabilities, and judging from measurements, I wouldn't expect a problem. Concerning the power supply, I'd recommend getting a larger one. You're right on the cusp with this behemoth. A 1200 watt power supply will insure you don't get random shutdowns and such... that said, you will have no problems booting up with an 850, but not recommended. I had to upgrade my power supply when I got mine (coming from a 750), and the Corsair HX1500i was a good fit for me. Hope that helps!
