What is the monthly or annual fee?

Most cloud services are free up to a certain gigabit level. I use dropbox as my cloud service. I have 8 gigs of free storage. If you want more you can use other cloud services such as box, skydrive, and google drive.

If I scan to dropbox or other non-Neat service, is there still a monthly fee to Neat?

NO, there is  no monthly fees for all cloud services other than neat cloud. Neat cloud is not only a box to save all your files. it can creates financial reports for you. 
I have been using the neat cloud service for less than 2 months now, and I am very happy with it. I am thinking of buying their service after 3 months of trial because it is really convenient.

I would like to know if I can use this for more than one business at a time as I do accounting for 3 companies and this would save me lots of time?

I think you can if yo are going to save the file to different destination. for example to Neat cloud. to Google Drive, To dropbox, Evernote,  or any cloud services. It will need lots of clicking on the screen though to switch to different cloud services. If you connect to your computer then you can choose your destination folder like any other scanner. does it work with windows 8?

I am use in a Mac but you just scan to the cloud then pull it up from the neat cloud I think it should work fine and if you have a smart phone  there is a app for it
