Would you trust this to hold a 120mm water cooler radiator on a 90mm exhaust port?

Yes, It seam to be well made. Whatever you mount to it make sure that you use the same type and size of screw. It it plastic. Use the smallest that will fit the hole. It will take a bigger one, but then you will have to use that size the next time you mount something. I use mine to direct cool air from the bottom of my case directly on my GPU.

Can this be used to place a 92mm fan on 120mm fan holes?

if you are trying to put a 80 where a 120 fits or vise versa this is what you want its made of thick plastic and should hold the fan in place for a long time with out vibrating loose. 
Also check that you have room to mount it with out hitting other areas[ MB, side of case,video card,]
Can also be used to channel air from a 120mm to a more precise point.
