Yes, it does come with a 3.5" HDD bay adapter, & mounting hardware, & even an OCZ sticker, just like the new ones. I didn't use mine because I bought it for a laptop install, so it slipped right in.. Now My laptop has a 7 second boot to OS!!! This is a wicked fast SSD! I was weary about buying refurbished as well, but I built a desktop last year with a Vertex4, along with an OCZ PSU, & it has been screaming fast, so took the chance on the V3... I've been using the Vertex3 for a few months now, & couldn't be happier!! I basically turned a $299.99 refurbished laptop, into an ultra book overnight, with this badboy!
