Does this product come with both smooth tops as well as picatinny as pictured?

I can't find original package, nor smooth tops, but I believe they come with both styles. I used Picatinnies on both rings and have reflex mounted to one.
From Burris site: "Includes both smooth and Picatinny ring tops". 
I've got QD version. Rock-solid mount! Highly recommended!

Hello, is it a QD (levers based) unit, or nut based? I'm interested in the lever-based mount, but you have both pictured.
