There is apparently a single 19-pin connector for a motherboard's USB header for this device's four ports. Most motherboard specs say that each header supports two ports. What does this mean? Will all ports work reliably? At the same time? Does it mean any two ports can be used at one time? Does it mean that only some combination of ports will work at one time?

It has been a while since I purchased this product, but I don't recall any limitations on using the ports, although we have never tried to stuff them all at once.
I suspect it has a built in USB hub of some sort, there is a chip on there.

In any case I have no complaints about its functionality.

Personally If all I want is a bunch of USB ports, I go for the external units.

I have two on my machine with a total of 14 usb ports.  They are conveniently locates on my desk.
I use an older USB2.0 one for all my software dongles, and wireless transceivers, and a 3.0 version that I use for everything else.

If you are trying to do VR and need a bunch of ports, you would be better off with a dedicated  multi port card, which I have also done.
