Yes, there is a specific step within the assembly instructions that makes sure the all three bottom monitors will be aligned.

Is the fourth monitor mount (the TOP center one) removable, so you could use it for three monitors and not have a pole sticking out?

Yes, that's how I've been using it.

Will this hold a 29inch monitor on the top and middle if the two side monitors are verticle?

The box states it will hold up to 27" monitors, so I doubt it.
