How is it cooled? It doesn't appear to have a fan, but I can't tell for sure.

It does not have a fan. Therefore it would normally just be cooled by the cabinet fans. I have purchased seral of these, but better units are available with cooling fans. This is and older design I only purchase as replacement parts, not upgrades.

What kind of inputs are on this card? hdmi?

The bulleted list says it has 2 diplay ports. These would be outputs, of course.
